Lobar Pneumonia Associated wih Myocarditis

Abstract

|
An autopsy case of lobar pneumonia associated with diffuse, non-suppurative and interstitial myocarditis was presented in a 23 years old Korean male.
At autopsy, the heart was found to be dilated. It was soft and friable, Myocardium was mottled with yellow, gray or both. Histologically, the lesion revealed circumscribed or diffuse inflammatory infiltration associated with cloudy swelling or active necrosis of myofibers and varying degree of fatty degeneration. Corresponding endocardium was free of inflammatory cells. The inflammatory process was patchy in distribution and the intensity of the process varied from one section to another. In instances of less intense involvement, it was not uncommon to find areas of myocardium in which inflammatory changes were minimal.
Inflammatory cellular response was predominantly mononuclears. In the most cellular areas lymphocytes outnumbered the other elements, which included large mononuclears and eosinophilic leukocytes.
The right lung was entirely consolidated showing gray-white appearance. No areas of abscess, hemorrhage or necrosis were encountered. Pleural fibrosis was rather prominent. Microscopically, the lung remained in stage of gray hepatization of lobar pneumonia.
